Armani clocked in @ 9:58pm. He was not in full uniform. He was missing his hat/visor and had his hoodie up the whole 2 hours viewd.
Lourita clocked in @ 10:24pm. She was in full uniform.

Hours viewed – 11:00pm-1:00am

11:00pm – There are no customers in the restaurant. Lourita is standing at the register looking at menu items. Armani is sitting at the break table with his hood up and eating food he brought with him. He is not on break. An online order comes in. (DD Order #999-43272)

11:01pm – Lourita goes to the kitchen to work on the online order.

11:02pm – Armani finishes eating and stands at the register with his hoodie still on.

11:03pm – A customer enters. Armani greets him and lets him know it is takeout only and the man leaves. Armani leans at the register, checking his phone.

11:06pm – Lourita packs the online order and sets it on the counter. She then goes outside for a smoke break.

11:10pm – Lourita returns from her smoke break and goes to the restroom.

11:12pm – Another online order comes in. (DD Order #999-43273) Armani goes to the kitchen to work on the order.

11:13pm – Lourita returns from the restroom, goes to the register, and rings in an employee discounted meal for herself. (Order #001-108862) She then goes to the kitchen and packs the condiments for the online order.

11:16pm – Lourita restocks the sliced cheese in the cooler.

11:19pm – Lourita helps packs up the online order.

11:20pm – A delivery person comes in to pick up one of the online orders. Armani hands him the order he just packed up and goes to get the beverage for the order. Lourita begins making her order.

11:22pm – Armani returns to standing at the register, checking his phone.

11:29pm – Customer 1 enters and goes to the register. Armani rings in a takeout order for her. (Order #001-108863)

11:30pm – Lourita starts to plate her order. She then begins making the order for customer 1. Armani processes the customers card payment then she sits at the counter to wait. Armani returns to leaning and checking his phone.

11:35pm – Lourita begins eating her meal in the kitchen. Armani is standing in the kitchen doorway waiting on the takeout order.

11:37pm – Lourita packs up the online order and hands it to Armani. Armani takes it to customer 1 and she leaves. He then returns to leaning at the register on his phone. Lourita continues eating her meal in the kitchen.

11:42pm – A delivery person enters to pick up the remaining online order. Armani hands him the order and the dasher leaves. Lourita is still in the kitchen eating and checking her phone. Armani returns to leaning at the register and on his phone.

11:45pm – Lourita packs up the rest of her food in a to-go box and sets aside on the kitchen counter. She rinses her plate off then goes to sit at a table near the windows, on her phone.

11:51pm – Customer 2 enters and sits at the customer counter. Armani greets him and rings in his order. (Order #001-108864) Lourita gets up, goes to the kitchen, and works on the order.

11:53pm – Customers 3&4 enter together and sit at the counter with customer 2. Customer 2 goes to the register to pay for his order. Armani processes his card payment and he sits at a booth to wait for his food.

11:54pm – An online order comes in. (DD Order#999-43274) Armani stays at the register chatting with customers. Lourita is working on orders in the kitchen.

11:56pm – Customer 5 enters and goes to the register. Armani greets him and rings in his takeout order. (Order #001-108865) Armani processes the customers card payment right away and the customer sits at a table to wait for his order.

11:59pm – Customers 3&4 are now ready to order. Armani rings in their order. (Order #001-108866) Lourita packs up the takeout order for customer 2 and takes it to him. She returns to the kitchen to work on remaining orders.

12:00am – Customer 2 opens his order and begins eating it.

12:01am – Customer 3 goes to the register to pay for his order. Armani processes his cash payment and the customer returns to sitting at the counter.

12:02am – Armani goes to the kitchen to help Lourita with orders. Lourita packs the order for customer 5 and takes it to him. The customer leaves.

12:03am – Lourita returns to the kitchen and packs up the online order and sets it on the counter. Customer 2 closes up his food and leaves.

12:04am – A man enters and joins customers 3&4 at the counter to talk. Possible undesirable as customer 3 gave him money.

Bloodhound Security & Investigations has access to the client's employee handbook policies and required training procedures and considered the applicable materials in evaluating these observations.

• Armani was not in full uniform because he was missing his visor/hat and wore his hoodie up while working, which conflicts with the uniform and appearance rules.
• He sat at the break table eating food he brought with him while not on break, instead of staying productive and using only an approved break area/time.
• He repeatedly checked his phone at the register and while standing around on duty, which is not allowed except during designated breaks and out of customer view.
• The report shows several periods where he was leaning or standing around at the register instead of remaining actively engaged in customer service.
• Armani was observed working on 3rd shift during the 12-2 am meal-break window, so any employee meal timing must be handled within that window and in the designated meal area.

Prior similar warning history: A prior warning report for Armani documented similar conduct involving standing around, phone use, and a meal-related issue on shift.

Signoff Action: Written warning prepared for employee signoff.
